A front end rejection from your DME carrier, essentially means the claim has an error of such severity that the claim cannot be processed as it is. Thus the system is rejecting it. Some issues as problems with the Common Working File (wrong ID or sex or DOB), or wrong HCPCS or ICD10 come to mind. This would result in the carrier being unable to process the claim as it is.
The problems could come from your end if you are submitting them directly to your DME MAC, or they could be coming from your clearinghouse. The latter would happen if they take your data and somehow incorrectly process what you send and then send that wrong information to the DME MAC. Hence the clearinghouse would be getting rejected.
If this problem persists, my suggestion would be to contact your DME MAC and speak with a level 2 supervisor along with a representative from your clearinghouse. I am working with someone with a similar but more definitive issue now. So stay tuned for more information on this.
